Results of Dengue Preventative activities in Northern Region in 2010
          The  year 2010 was the second year after the national dengue control and prevention program which was restored and re-operated. 28/28 provinces and cities in the north of the country were still in the period of completing the network of dengue control and prevention. 

          Since 2009, In key areas, service areas, especially in Hanoi, Nghe An, Ha Tinh, Thanh Hoa, Nam Dinh the active epidemiological surveillance and prevention of Dengue fever have been implemented actively and positively, the stamping service measures have been carried out promptly and efficiently. The outbreaks have occurred in communes/wards at small and medium-scales only. The activities of the project have developed comprehensively and effectively, as follows:

     1. Epidemiological surveillance and DHF situation


       Patient surveillance:
          - 4,316 communes/253 districts in 28 provinces/cities in the North implemented patient surveillance in accordance with guidance of MoH.
          - Northern region had a total of 5,550 clinical cases, decreasing 70% in comparison with 2009 (18,485 cases), and no death.  
          - The provinces with high number of dengue cases were Ha noi (3,152 cases; 56.8%); Ha Tinh (933 cases; 16.8%), Nghe An Province (725 cases; 13%); Thanh Hoa (265 cases; 4.8%).
          - Among 5,550 cases, 1,999 cases were DF(36%) and 3,551 cases were DHF (64%).
          - The number of cases focused on adults and children over15 years old (87%).
 
       Virus surveillance:
          In 2010, 1,067 serum samples were tested (19.2% total of Dengue suspected cases, this percentage was 7% higher than the plan). 271/1067 samples were positive (24.5%). 303 samples were isolated viruses (5% total of dengue cases, over fulfill norms to 3%), 138/303 samples were positive to dengue viruses (45.5%). In 2010, northern region, all four types of Dengue viruses were circulated, but D1 was dominant (63%), followed by D2 (16.7%), D3 (16.7%) and D4 (3.6%).
 
       Vector surveillance:
          Vector surveillance index: 140 sentinel sites in 8 provinces/city type A, B of the region have routine monthly report. Results from the system showed that vector indices (Breteau index, Mosquito density index (MDI) increase from August to Octorber. Breteau index and the highest number of mosquito density in Nghe An in August (Breteau: 100; MDI: 1.7). In Hanoi, MDI remain low from 0.01 to 0.07 for 12 monitoring month. Some provinces like Thai Binh, Bac Ninh, Hai Duong only detected the presence of Aedes albopitus.
 
          Results of monitoring of vector susceptibility to chemical insecticides in 8 provinces and major cities in 2010 (Nghe An, Ha Tinh, Thanh Hoa, Nam Dinh, Vinh Phuc and Hai Phong, Thai Binh) in the strip by WHO showed that Aedes aegypti was still sensitive to Deltamethrine 0.75%, Permethrin 0.05%, Lambdacyhalothrin 0.05%.
 
       Epidemic prevention activities:
          NIHE keep monitoring and send working group to DF outbreak to support, guild and directly handling the outbreak with PMC of Provinces and districts. NIHE also provide chemical, equipment and budget to province. Especially, NIHE formed 6 groups to go to supervise and monitoring outbreak response activities in province which have outbreak and flood affected province.
 
          Province PMC have strong coordinate with NIHE, mobilize the whole systems include politic, social and difference organization to help control the outbreak. The health service of Provinces directly guide unscheduled meeting with all health facilities on site to control the situation, monitoring and handling outbreak. Send health worker to outbreak site, flood site to completely guild treatment, surveillance, prevention, especially environment treatment, kill vector, self hygiene and outbreak prevention after flood.
 
          In 2010, generally, the northern region has successful control DF, no big outbreak occur and fulfill the objective of the year.
 
     2. Other project activities
 
       - Regarding the organization and direction:
          Organizational system and steering committees of the project were established at all levels, from center to communes. Maintaining the meetings of steering committees monthly and quarterly to assign tasks, make plans, report the progress and results to supplement and adjust activities of the project. Consolidated the steering committees of the project at provincial, district and commune levels. Responsible staff of provinces, districts and communes were trained and improved the knowledge as well as skills of surveillance, control and treatment of DF/DHF. In 8 key provinces/cities, the collaborator network was restored and retrained on knowledge and skills to be able to work effectively.

          Promulgated 26 documents, official dispatches to direct all activities of dengue control and prevention. Directed the local areas to strictly implement the documents, policies of Government and Ministry of Health. Especially, the official dispatch No 1668/CĐ-TTg dated on 18/9/2010 of Prime Minister was implemented strictly and effectively.

       - Communication and education:
          Project Steering Committees coordinated with the General Department of Preventive Medicine to make a propaganda film on dengue. They also provided 400,000 propaganda hand posters on dengue for 28 cities and provinces in the North. Books were Printed and distributed for collaborators in 8 key provinces in Northern Region. 2,500 propaganda CDs of dengue control were copied and distributed. Communication and education activities had been implemented in various forms in the provinces/cities in Northern areas, specifically 604 local broadcasts, 59,545 broadcasts broadcasted in local radios and televisions; 20,963 posters, 24,112 slogans and 685,247 hand posters distributed to health facilities and communities.

       - Trainings and Conferences:
          Organized the conference on resistance of Aedes to insecticides and it’s impacts to control and prevention of dengue; updated the information of chikungunya and dengue haemorrhagic fever in Southest Asia; statistic management through space by Google earth software.

          Provided training on «How to handle outbreaks of dengue» for officials in 28 provinces and cities to strengthen and improve surveillance, detection and handling of the dengue outbreak in routine in accordance with guidance of the Ministry of Health.


          The Steering Committee coordinated with Hanoi Preventive Medicine Center in organizing training courses to improve dengue surveillance and prevention for health care workers in 29 districts of Hanoi; Provincial Preventive Medicine centers of the provinces and cities coordinated with the Medical Centers of districts in organizing 379 training courses for responsible staff of provinces and districts with a total of 25,152 trainees; 1,790 commune training courses  with a total of 6,512 staff of local authorities, 7,932 teachers and 95,246 pupils of primary and secondary schools, and 8,740 collaborators.
 
       - Supplies, equipment chemical:
          In 2010, the project provided 557 liters of chemicals Delta 2.5EW UK; 1000 L and K-04 2EW Othrin ULV sprayer for the provinces of Ha Tinh, Nghe An, Thanh Hoa and Nam Dinh.

       - Monitoring, evaluation: 
          Strengthened direction, inspection and monitoring of project implementation in the northern provinces. The regional steering committee organized 28 inspective teams to monitor, inspect and speed up project implementation before, during and after epidemic reason in 8 key provinces/cities of Hanoi, Nghe An, Thanh Hoa, Ha Tinh, Nam Dinh, Hai Phong, Thai Binh and Vinh Phuc.
 
          Monitoring of patients, serum, vector had been consolidated and stable operation at defensive health centers and provinces, with close coordination with district officials, in implementation as well as its role to direct and supervise professional, implementing preventive measures against dengue at key points.

       - Reward
          Northern Region offered rewards to 42 teams and 40 individuals who obtained many achievements in the control and prevention of dengue in 2010 in order to encourage the spirit of individuals and units to continue upholding the spirit of responsibilities, spirit of services, and to strive for higher achievements in the coming years.
